See the whole transition
Bring appointments, deadlines, referrals, household needs, and caregiving responsibilities into one view.
Golden Rose Wellbeing
Care Ahead Roadmap
A health transition can leave you with instructions, appointments, questions, and responsibilities—but no single plan connecting them.
Care Ahead Roadmap helps adults and caregivers understand their care, organize what matters, prepare informed questions, and keep hold of the practical steps affecting their wellbeing.
For adults physically located in Arizona during online sessions. Care Ahead is nonclinical and private pay.
The space between appointments
Healthcare and social-service systems contain valuable knowledge and resources, but responsibility is often divided among professionals, facilities, benefit structures, and separate encounters. The person receiving care—and the people helping them—may be left to carry information across those boundaries and determine what happens next.
With Care Ahead, the separate pieces become one practical view of the transition.
How it helps
We organize the information you already have and turn it into a usable plan—so you can participate more confidently without giving up decisions that belong to you or your licensed healthcare professionals.
Bring appointments, deadlines, referrals, household needs, and caregiving responsibilities into one view.
Separate practical actions from questions that need to return to a physician, nurse, pharmacist, therapist, or other professional.
Identify who owns each next step, when it should happen, and what backup may be needed.
Identify relevant community, transportation, caregiver, social-service, and educational resources to explore.
What you receive
Care Ahead is designed for a defined health transition—not an open-ended coaching relationship.
A video conversation focused on your transition, practical concerns, responsibilities, and questions.
A personalized electronic plan that preserves important information across appointments and settings, with priorities, responsibilities, questions, resources, and next steps.
A 30-minute video meeting to review progress, practical barriers, and remaining priorities.
Payment is not currently being accepted. Interested clients will receive complete terms before deciding whether to participate.

Who it may help
Clear boundaries
Golden Rose Wellbeing does not provide nursing or medical assessment, diagnosis, treatment recommendations, clinical triage, medication advice, psychotherapy, emergency care, or a nursing plan of care.
We can organize instructions you have received, but the appropriate licensed professional must explain or clarify their clinical meaning.
For an emergency, call 911. For urgent medical questions, contact the appropriate treating professional or urgent service. Call or text 988 for the Suicide & Crisis Lifeline.
About Golden Rose Wellbeing

How we approach the work
“A person’s wellbeing is shaped by what happens before, between, and after healthcare encounters—not only by what happens in the examination room.”
